Thyroid Fine-Needle Biopsy

Fine-needle aspiration biopsy (FNAB) takes a thin-needle cell sample from a thyroid nodule under ultrasound guidance. It is the most decisive test for telling benign nodules from those needing further action.

Short Answer

FNAB is offered when ultrasound features and nodule size cross the recommended threshold. The result is reported on the Bethesda I–VI scale and guides follow-up versus surgery.

When Is Biopsy Indicated?

Decisions follow TIRADS-based thresholds. Suspicious features (hypoechoic, taller-than-wide, irregular margins, microcalcifications) lower the size threshold for biopsy.

How the Procedure Is Performed

Under ultrasound guidance, a very thin needle is passed into the nodule and a few passes are taken. The procedure is quick and well tolerated; a local anaesthetic is rarely needed.

Treatment Options

  • Repeat biopsy in non-diagnostic (Bethesda I) results
  • Follow-up for benign (Bethesda II)
  • Re-evaluation in indeterminate (Bethesda III–IV)
  • Surgical planning in Bethesda V–VI

When Is Surgery Considered?

  • Bethesda V or VI
  • Persistent indeterminate result with suspicious ultrasound
  • Patient preference together with clinical risk

Surgical Methods

  • Diagnostic lobectomy in selected indeterminate cases
  • Total thyroidectomy when biopsy is malignant and disease is bilateral

Preoperative Preparation

  • No special preparation; eat and drink as usual
  • Inform the team about blood thinners
  • Bring previous ultrasounds and biopsies

Postoperative Follow-up

  • Mild pressure for a few minutes
  • Brief soreness or bruising at the puncture site
  • Result is usually ready in a few days

Risks and Safe Surgery

  • Small bruise or pain at the puncture site
  • Rare bleeding
  • Non-diagnostic sample needing repeat

Process in Antalya

FNAB is performed where indicated and the Bethesda report is interpreted together with ultrasound and clinical findings in Antalya.

When to Seek Urgent Care

  • Rapidly enlarging neck swelling after biopsy
  • Severe pain or breathing difficulty

Frequently Asked Questions

Discomfort is minor and brief. Most patients tolerate it without local anaesthetic.

Share Your Reports for Initial Guidance

Hello, I would like to request a pre-evaluation for thyroid/goiter/thyroid nodule. I can share my ultrasound, biopsy and blood test results.

This information is provided for general patient education only and does not replace diagnosis, treatment or a surgical decision. Personal evaluation requires a clinical examination and the necessary tests.
