Neck Lymph Node Dissection
Neck dissection removes the lymph nodes most likely to contain thyroid cancer. It is planned alongside thyroidectomy when biopsy or imaging shows nodal disease, or when risk features warrant central dissection.
Central dissection is added in selected total thyroidectomies; lateral dissection is added when lateral neck nodes are proven to contain cancer.
Central Neck Dissection
Removes lymph nodes around the trachea (level VI) and is performed for proven or high-risk papillary cancer.
Lateral Neck Dissection
Removes lymph nodes along levels II–V on the side of proven disease, preserving the spinal accessory nerve, internal jugular vein and sternocleidomastoid muscle where possible.
Treatment Options
- Central neck dissection (selected)
- Lateral neck dissection (proven disease)
When Is Surgery Considered?
- Biopsy-proven lateral neck disease
- Selected high-risk papillary cancer
Surgical Methods
- Anatomical, structure-preserving technique
- Drain when needed
- Careful identification of recurrent laryngeal nerve, spinal accessory nerve and great vessels
Preoperative Preparation
- Detailed neck mapping with ultrasound and CT
- Vocal cord assessment
- Anaesthesia and ENT review
Postoperative Follow-up
- Drain managed in the early days
- Physiotherapy for shoulder after extensive lateral dissection
- Wound care and scar guidance
Risks and Safe Surgery
- Shoulder discomfort or weakness
- Numbness in the neck/ear region
- Lymphatic leak (uncommon)
- Bleeding, infection
Process in Antalya
Imaging, biopsy of lateral nodes when needed and ENT review are coordinated; the operation is planned alongside thyroidectomy.
When to Seek Urgent Care
- • Sudden neck swelling
- • High-volume drain output
- • Severe shoulder dysfunction
